Please see the accreditation process in detail for more information. The sections below note when each form is used:

Registration

When you have decided which named award(s) to seek accreditation for, you should complete the Registration Form.

Accreditation

As you prepare for accreditation your mentor will support you to put together some documentation:

  • A SEDA-PDF Institutional Mapping Document (one document for the institution and all programmes).
  • See Annotated Institutional Mapping Document for a version with pointers as to how it should be completed. (When adding a new programme to an existing accredited provision, simply update your existing institutional mapping document).
  • A completed Programme Mapping Document for each programme.
  • See Annotated Programme Mapping Document for a version with pointers as to how it should be completed.
  • Both mapping documents will be accompanied by appendices of institutional and programme documents. Your mentor will help decide on these e.g. relevant institutional strategies and student facing programme documents (e.g. handbooks).

Recognition

Following the accreditation event, the accreditors will complete a Recognition Report Form which will go to the PDF Committee for ratification.

The report form will include any conditions and recommendations. Responses to these and feedback/reflection from the institution will be completed on the same form.

First year Report

One year after accreditation, a First Year Report Form will be required for all newly accredited programmes.

Review

One year prior to your accreditation review you will be asked to complete a Pre-review Report Form.
NB you should also update the institutional mapping and programme mappings forms.

Programme Modifications

If you need to make any minor changes to your accredited programmes within the accreditation cycle, submit a Minor Modifications Form.
